Job Description

Cerner Corporation is current seeking a full time Consumer Advocate for our Health Exchange department!

As a Consumer Advocate, you will engage a population of members in their care, assist them in navigating the health care system and provide support, encouragement, and education. You will support members' adherence to their individual care plan, assisting to identify and remove logistical, emotional, or social barriers to treatment, monitor member-reported biometrics, medication adherence and reported challenges or barriers and follow chronic disease management protocols to connect members with the appropriate resources or notify their care teams for follow-up. You will also facilitate member referrals to services and resources, including member assistance programs, community-based services, and mental health support, assist providers' offices with member-specific missed appointments through outreach and scheduling and provide customized, evidence-based member education in areas including weight management, exercise, tobacco cessation, stress reduction and chronic disease self-management. Lastly, you will assist with the coordination of care across the care continuum, helping members understand how to identify and access care in the most appropriate setting. Join Today!

Working Environment

Hybrid & Onsite: No Relocation Assistance Offered

This position offers a Hybrid & Onsite working environment. Meaning you will split working time between a Cerner office or client site and remote. If your current geographical location requires relocating, Cerner will not offer relocation assistance to the specified location. #LI-Hybrid

Qualifications

Basic Qualifications
  • At Least 4 years total combined related work experience and completed higher education, including:
    • At least 2 years customer service, call center and/or hospitality experience
    • At least 2 years additional work experience directly related to the duties of the job and/or completed higher education

Preferred Qualifications
  • Associate degree
  • Licensed Practical Nurse – State Board
  • Certified Medical Assistant (required where available)
  • Certified Community Health Worker – State Board
  • Certified Nurses Adie – State Board

Expectations
  • Willing to work additional or irregular hours as needed and allowed by local regulations
  • Work in accordance with corporate and organizational security policies and procedures, understand personal role in safeguarding corporate and client assets, and take appropriate action to prevent and report any compromises of security within scope of position
  • Perform other duties as assigned.
